So what do we mean by a "hardship"? Well, if this levy is causing you a hardship where you're having trouble paying for your BASIC monthly expenses, select Yes.

Question 4: Approximately how much do you owe to the IRS?(Required)

Question 5: Is this an income tax liability?(Required)

The most common answer here is Yes. An income tax liability is basically the tax debt from your tax return (IRS Form 1040) that's due every April 15th. If you're not sure, that's OK too.

Question 5b: Is this a payroll tax liability that has been assessed against you personally?(Required)

Basically, have you had payroll taxes assessed against your social security number. If so, select Yes.
